I lost my green card last year and now I want to get the replacement. The green card had my maiden name on it, and now that I'm married I would like to get the new last name on it. My question is which option do I need to choose on the I-90 form:

- green card lost of stolen or
- change of biographic information

The form requires me to choose only one option, so how do I go about that? Also what fee do I need to pay - $370 incl. fingerprints fee?
