We have recently returned to the UK after a lovely two week holiday in New York and Boston, during our visit our green visa waiver forms have been misplaced so we could not hand them in upon our departure and have not located them since.
We would love to visit USA again in the future so need to get this issue resolved.
We have read that we need to contact the USA immigrations department and have the following information to send them

Departure boarding passes
Departure luggage labels
Departure duty free purchase receipt
Credit card statement with UK purchases since our return
Copies of our passports

Will this be enough information?
Will we get confirmation from the immigrations department that our records have been closed?

Any help would be appreciated
